Why Do Dark Marks Remain After an Acne Breakout Heals?

A blemish may clear in days, yet the brown or gray-brown spot it leaves behind can linger much longer. This is often post-inflammatory hyperpigmentation, or PIH, rather than a permanent acne scar. At Skin MD and Beyond in Plano, TX, Fiona Wright, MD evaluates skin pigmentation after acne and other inflammation. Dr. Wright has practiced in Plano since 1996 and completed postgraduate dermatology training through Cardiff University. The skin may feel smooth even though its color has not returned to baseline.

Why Does Acne Leave a Dark Mark?

Inflamed acne can stimulate melanocytes, the cells that make melanin. After the swelling settles, extra pigment may remain in the area and create a flat mark that stands out from the surrounding complexion.

Some skin tones are more prone to PIH, and the depth of the pigment can influence how long discoloration remains visible. Picking or squeezing a blemish can add inflammation and make the leftover mark more noticeable. Sun exposure can also deepen existing pigmentation, which is why consistent sun protection matters while the skin is healing.

Is It Pigmentation or an Acne Scar?

A dark spot and a textural acne scar are different concerns. PIH changes skin color without creating a depression or raised area. Acne scars can leave pits, divots, or other texture changes after the original lesion heals.

That difference matters when choosing care. Our acne and acne scar care considers active breakouts, lingering color changes, and true scarring separately rather than treating every post-acne mark the same way.

How Can Post-Acne Pigmentation Be Treated?

Some marks fade gradually as the skin renews itself. When discoloration persists, Dr. Wright can assess whether the pigment is superficial, deeper in the skin, or mixed. A treatment that works well for surface pigment may not reach a deeper concern.

Depending on your skin and history, options at our office may include chemical peels, microdermabrasion and DermaSweep-MD, or a broader complexion correction plan. Patients who are prone to PIH need particular care around procedures that create inflammation or heat. Dr. Wright has experience treating PIH as well as evaluating patients who may be more likely to develop it.

When Should a Post-Acne Mark Be Evaluated?
